Witness Testimony Agreement Form

This agreement allows individuals to complete witness testimonies on behalf of learners.

This agreement is valid for 18 months from the signing date.

Please note that all signatures included in any witness testimony must be included on a Witness Testimony Agreement Form.

Please note that we may request further evidence of occupational competency, where required.

If you are acting as a Witness, you must:

1. Be working in a suitable role to verify the evidence.

2. Hold a qualification in the specialist area, at or above Diploma Level 3 in Health and Social Care (or equivalent) and have a working knowledge of the RQF units.

3. Be occupationally competent in their area of expertise

Purple Ledge Ltd as the training provider, will require a copy of the certificates to confirm the qualification level held by the person acting as a Witness.
